To maintain healthy blood pressure, avoid alcohol, quit smoking, and manage your diet.

Your everyday choices can save you from lifelong medication.

What is an ACL Injury? – Dr. Nawaid Ahmed Explains

In this video, Dr. Nawaid Ahmed explains what an ACL (Anterior Cruciate Ligament) injury is, using a knee joint model for better understanding.

This is the human knee joint. The front part is the kneecap, and the joint is supported by four main ligaments that prevent excessive movement and provide stability.

Two of these ligaments are located on the outer sides the LCL (Lateral Collateral Ligament) and the MCL (Medial Collateral Ligament). The other two are located inside the joint — one of which is the ACL, and the other is the PCL.

Here, you can also see the meniscus, which are two shock-absorbing pads (shown in blue on the model). They cushion the bones and reduce friction during movement.

An ACL injury occurs when the anterior cruciate ligament is torn or detached, either from the bone or within the substance of the ligament itself. This is commonly seen in sports injuries, especially in athletes involved in high-impact or twisting motions.

When the ACL is torn, the knee can become unstable, making the thigh bone (femur) slide over the shin bone (tibia). The patient may feel as though the knee is ‘giving way’ or unstable, especially during walking or running — this is referred to as an unstable knee.

Treatment depends on the severity of the tear and the patient’s activity level. In active individuals or athletes, arthroscopic ACL reconstruction surgery is often recommended to restore stability and function.

Do All ACL Injuries Require Surgery? – Dr Nawaid Ahmed Explains

When the ACL (anterior cruciate ligament) is torn, it usually cannot heal on its own, especially if it’s torn in the middle. In such cases, we often need to surgically reconstruct the ligament, meaning we create a new one using a graft. Repairing it with medicines alone is not possible.

However, if the ACL is torn from where it attaches to the bone, like the lower bone (tibia) — and if the tear is clean, a repair may be possible in selected cases.

There is a small group of patients with a partial ACL tear or those whose hamstring muscles can compensate for the ligament. In these cases, we sometimes try non-surgical treatment with physiotherapy as a trial.

But in most cases, a full ACL tear requires surgical reconstruction to restore knee stability and prevent long-term issues.

When Should You Consider Knee Replacement Surgery? – Dr Nawaid Ahmed Answers

Knee replacement surgery should be delayed for as long as possible. We usually recommend it only when pain becomes so severe that it affects your daily life and you’re unable to think beyond the discomfort.

Since this surgery is mostly done in older patients, our goal is to postpone it as much as we can, until it becomes absolutely necessary for improving quality of life.
